Wish Application Form

Wish Qualification

As part of our commitment to continually improve and enhance our wish-granting capabilities, we have implemented a new referral model, therefore we have made some changes to the types of applications that can be submitted.

From 1st July 2024, our agreed Community Referral Partners will now be able to submit a referral for a child or young adult who falls within Make-A-Wish UK’s eligibility criteria. To read more about our Community Referral Partners, please head over to our webpage: Community Referral Partners

We continue to accept all referrals for children with a prognosis of 12 months and under, which can be made by a member of the child’s health, social care, or educational team.

If you would like to know more about our referral processes, please contact a member of the team on : 0118 3043368 or email:
referrals@makeawish.org.uk

Unfortunately we are currently unable to accept a referral for a child or young adult who is not a UK resident. To check whether there is a Make-A-Wish in your residing country, please go to https://worldwish.org/around-the-world/

For us to be able to accept a referral, we will need confirmation that the child falls within the Make-A-Wish UK eligibility criteria. If a child does not fall within our medical criteria we will be unable to explore a wish. If you have any questions regarding eligibility criteria please contact a member of the team on referrals@makeawish.org.uk or 0118 304 3368.

Unfortunately we are currently unable to accept any non-urgent referrals. For more information please contact a member of the team at referrals@makeawish.org.uk or 0118 304 3368.

For us to be able to accept a referral, we will need confirmation that the family are aware your are making the application on their behalf. Please submit a referral once you have spoken to the family - if you have any questions please contact a member of the team at referrals@makeawish.org.uk or 0118 304 3368.

Unfortunately, if a child has already had a wish granted by Make-A-Wish or another Wishgranting organisation we are unable to grant another. If you have any questions please contact a member of the team at referrals@makeawish.org.uk or 0118 304 3368.

Unfortunately we are currently unable to accept any non-urgent referrals, other than from one of our Community Referral Partner organisations. If you are interested in finding out how to become a Community Referral Partner and would like further information please contact Fiona Carlisle, Wish Referral Partnerships Manager: Fiona.Carlisle@makeawish.org.uk

Please note, while we hope to grant all wish ideas, there is no guarantee that a specific wish can be granted until explored further with the Wishgranting Team. Unfortunately we will be unable to explore a wish outside of the UK if a child's prognosis is less than 6 months. Due to popular demand, we are currently unable to accept any further wishes to travel to Disneyland Paris. If you would like to discuss this further with a member of the Wishgranting Team please call 0118 3043368.
Unfortunately at this time, we are only able to accept non-urgent wishes from agreed Community Referral  Partners where the child would like to explore wishes for Lapland UK or our Disney UK wish experience. If this is an urgent application and a child has a prognosis of less than 12 months, please amend the above details and continue with the application.

By continuing with this registration process, you're agreeing to the Make-A-Wish UK terms and conditions and privacy policy. These documents explain where and how we'll use your data. Just ask if you have any questions.
Please answer all the available questions and if the submit button is not available to you this means your referral does not meet our criteria at this time.
