Wish Application Form

As part of our commitment to continually improve and enhance our wish-granting capabilities, we are implementing a new referral model. To facilitate this transition, we have made some changes to the types of applications that can be submitted.

We are delighted to be able to accept referrals for A Disney Wish, our exclusive Disney wish experience, taking place in Summer 2024. You can still submit any referrals for wish children and young adults who may need a wish to be granted urgently. This means, that at present, we will only be able to accept new referrals for children with a prognosis of less than 12 months.

Is the child currently a resident of the United Kingdom?

Does the child/young adult you are referring fall within the Make-A-Wish UK eligibility criteria?

Is the family aware you’re making this application on their behalf?

Has the child received or registered for a wish from another organisation?

Which organisation?

What is the child's prognosis?

Please select the wish type that the child and/or family have expressed an interest in exploring with Make-A--Wish UK.

Does the child require the use of mobility aids?

Unfortunately at this time, we are only able to accept non-urgent wishes for our Disney UK wish experience. If this is an urgent application and a child has a prognosis of less than 12 months, please amend the above details and continue to apply.

Are you working with/associated as a Community Referral Partner of Make-A-Wish UK?

Unfortunately, we are currently unable to accept any non-urgent referrals, other than from an agreed Community Referral Partner.
